On December 30, 2014, the covered entity (CE), Advance Rehabilitation & Consulting LTD, discovered that a port on one of its servers was publically accessible to the Internet and allowed an automated botnet attack to the server. Internal investigation revealed that one spreadsheet from 2009 was accessed, but there was no way of knowing if the spreadsheet was viewed.
