On January 13, 2016, the covered entity (CE), Walgreens Pharmacy, reported that a theft took place at one of its stores located at 1350 Broadway in New York. The breach involved prescription numbers, first and last names, dates of birth, addresses, medication and insurance information for approximately 880 individuals.
