The covered entity (CE), Mattapan Community Health Center (MCHC), reported that an employee was the victim of an email phishing scheme that affected the electronic protected health information (ePHI) of 4,073 individuals. The ePHI involved included names, addresses, dates of birth, Social Security numbers, and medical information.
