The business associate (BA), Imagine360, reported that two of its vendors experienced a cyber-attack that compromised the protected health information (PHI) of 132,807 individuals. The PHI involved included names, addresses, dates of birth, drivers’ license and Social Security numbers, financial information, and diagnoses.
